Avastin, a drug for the treatment of metastatic breast cancer, has had its authorization suspended by Health Canada recently. Health Canada stated that the drug “may do more harm than good”, and has no positive effect on tumour size or life expectancy. Studies conducted after it was initially approved in February 2009 have shown that it is not safe and effective in the treatment of metastatic breast cancer, as negative side effects include heart attack, bleeding and tearing of tissues in the nose, stomach and intestines. These life threatening risks are the primary reason behind Health Canada’s decision, which was determined by a panel of doctors, scientists and patient advocates of breast cancer. The FDA governing pharmaceuticals in the US has also withdrawn approval.

Avastin, manufactured by Hoffmann-LaRoche, still has approval for the treatment of glioblastoma (a type of brain cancer) and other metastatic cancers: colon, rectal and lung.

If you are a breast cancer survivor, have you heard of Avastin?

Dr. Jugenburg is dedicated to helping breast cancer survivors reclaim their bodies by promoting breast cancer reconstruction. With just 9% of all breast cancer patients in Canada undergoing breast reconstruction, Dr. Jugenburg hopes to increase this number so that women can go on living, not just surviving. Earlier this year, he spoke at Living Pink at Crystal Beach, a breast cancer awareness and fundraiser event.

October is Breast Cancer Awareness Month, and the 19th was National BRA Day! BRA Day, which stands for Breast Reconstruction Awareness, is devoted to promoting breast cancer education and helping women understand their options following mastectomy. Did you know that breast reconstruction is covered by OHIP?

For BRA Day, Dr. Jugenburg spoke in Toronto at the Humber River Regional Hospital (Finch Site). His talk, entitled “Breast Cancer Reconstruction – Yes, You Can”, explored what breast reconstruction following breast cancer therapy could do for survivors.

One risk of breast augmentation is symmastia. Symmastia means one breast, which is why it may also be referred to informally as a unibreast or breadloafing. This happens when tissue that normally sits upon the sternum to form the cleavage lifts up. Without this “wall”, the implants slowly drift towards each other, forming the appearance of a single breast. Implants that are placed too close together, with little space between them, can also cause milder forms of symmastia. This can occur when the surgeon attempts to achieve a desirable amount of cleavage, but overdoes it.

Symmastia varies in severity, depending on the amount of space between the implants. It also depends on the perception of the patient.

The placement of the implants (submuscular or subglandular) determines the suitable method of treatment, but normally involves corrective surgery to reattach tissue in order to create a wider space between the breasts. Usually, all stitches are placed underneath the skin to prevent visible scarring and improve the chances of successful recovery. A specially designed bra with additional support between the breasts is worn to facilitate recovery (aka thong bra).

Symmastia can also occur from birth, where a woman’s own natural breast soft tissue grows together.

Silicone/saline breast implants, fat transfer, the Brava Bra or injectable filler – let’s be honest, if it were up to me, I’d definitely go for the injectable filler to increase the size of my boobs.

I admit it: I’m a wimp when it comes to surgery. I do not want to endure the surgery or the extensive recovery afterward, even though it’s currently the most effective way to get bigger boobs. There are just too many things to worry about, related to the surgery and the anesthesia.

As for fat transfer, there’s still much needed improvement on the technique and it’s not readily available anyway in Toronto.

And the Brava Bra – simply a cruel torture device that is best reserved for masochists who just want a small increase (let me know if I’m wrong).

So that leaves me with the “boob jab”, aka Macrolane. And I’m not alone – apparently a number of informal surveys have found that the majority of women interested in breast augmentation would opt for this injectable filler, too. Sure, it only lasts for about a year and has its risks (lumpiness and leakage), but the thought of a nonsurgical procedure that involves an injection alone to achieve bigger breasts is a great concept. I’m all about noninvasive!

Unfortunately, it’s only available in the UK and Europe right now. I wonder if the folks at Health Canada will approve it?

Plastic surgeons are warning that breast augmentation using transferred fat may not be a “two-for-one” solution for those who want to get a bigger bust and slim down at the same time. The technique also requires more research before deeming it “safe” and “effective” for the public.

While breast augmentation with implants is still considered the safest and most effective method, the fat transfer technique is currently being improved upon to make it a viable alternative. One improvement is injecting the fat into the fat of the breast, instead of into the tissue as was once done.

The benefits of this procedure over regular implants are results that may last longer and look more natural. However, complications such as the formation of cysts and fat necrosis still remain despite improvements to technique. Patients also typically require multiple procedures to achieve a desirable result in order to reduce the risk of complications occurring and for the body to accommodate the newly transferred fat. The amount of fat transferred from one part of the body to another is also not a lot, which means it won’t make a significant difference in the donor area (which is why it’s not a “two-for-one” solution, unless you only need a small amount removed from the to make a difference).

I just found out I’m pregnant. All I can think about is how my implants from breast augmentation surgery will handle the changes to my body. Can you tell me anything about this?

Congrats on the pregnancy! It’s fantastic news!

You’ll be happy to know that your breast implants will not be affected by your pregnancy. The only thing that will be affected is your actual body.

Pregnancy does involve a massive change to your body. It will need to adapt to a human being growing inside of you, which means that the size of your belly will increase dramatically and your breasts will also most likely grow as well as they prepare to produce milk. Your hormones can affect the size and shape of your breasts, possibly producing positive or negative changes.

Many women experience positive permanent changes in the shape and size of their breasts. Unfortunately, many others experience negative permanent changes which include droopiness and a decrease in size. However, there is no way of telling what will happen to your breasts right now. You’ll just have to wait and see – so embrace your pregnancy and deal with the changes after you’ve given birth and finished breastfeeding (if you plan to). At that time, you can see whether your breasts are still pleasing in appearance to you, or if they need further augmentation and/or a lift.
