How a Breast Augmentation Works

There are several factors to consider throughout the breast augmentation process. While each aspect will be discussed during your initial consultation, here at SixSurgery, our team focuses on empowering patients with the information they need to better understand the surgery.

Step 1
Type of Implant

This is where you get the option to choose between a silicone implant vs a saline implant. At SixSurgery, we use an equal about of both types, as the main difference is in the feel, not look.

Step 2
Size of Implant

When choosing a size for your implant there are many options to consider. These consist of the implant volume, profile, width, and shape.

Step 3
Incision

A small incision is made through careful placement. At our clinic, we can make the incision in either the breast fold (Infra-Mammary Incision), through the areola (Periareolar Incision), or the armpit (Transaxillary Incision).

Step 4
Placement

The placement of implants in breast augmentation can affect their appearance and feel. You can opt for subglandular placement above the muscle for a rounder look or submuscular placement below the muscle for a more natural vibe, with a dual plane option that combines both for a tailored result.

Step 5
Techniques

At SixSurgery, our skilled team offers a range of breast augmentation techniques tailored to your needs. Options include the Tiny Scar method for minimal scarring, fat transfer for natural results, and procedures for breast asymmetry or lift. We’ll guide you in choosing the best approach during your consultation to whiever best suits your needs.

Procedure Types

There are a few types of breast augmentation procedures, each tailored to achieve specific goals based on individual anatomy, desired outcomes, and lifestyle. At Toronto Surgery, we specialize in both Silicone and Saline implants.

Silicone
Saline
Flexible Shell

Silicone breast implants feature a resilient, flexible silicone shell filled with a thick, medical-grade silicone gel, offering a texture similar to natural breast tissue.

Different Size Options

Available in different sizes and profiles, silicone implants provide an excellent choice for achieving desired aesthetic outcomes.

Long-term Safety

While silicone implants once faced safety concerns, today’s implants are designed with a cohesive gel that holds its shape, earning them the nickname 'gummy bear implants.

Natural Look

Filled with a cohesive silicone gel that closely mimics natural breast tissue, these are popular for their natural feel and stability.

Safety of Saline Implants

Saline implants are safe and natural as they are filled with saltwater, which naturally exists in the human body.

Variety of Profiles

Available in a wide variety of shapes, sizes, profiles and textures, there’s a saline implant to meet the needs of a vast array of surgical goals. Saline implants have the unique benefit of being filled with fluid after the implant has already been inserted under the breast tissue, allowing them to be placed through the smallest scar possible. This means the size of the implant does not influence the incision and ultimately the scar.

Durable shell

While the implant is filled with saline, the shell is made with highly durable silicone.

Am I A Good Candidate?

Genetics

Some women have breasts that may appear smaller or less proportional to their body, which is entirely natural and common. If this affects how you feel about yourself, breast augmentation could be a positive option to help you feel more comfortable with your appearance.

Disproportionate or Uneven Breasts

You might feel that your body appears out of proportion or that your breasts are unevenly positioned, creating asymmetry. Breast augmentation can help create a more balanced, harmonious look, enhancing both confidence and satisfaction.

Weight Loss

Changes to your health and fitness can sometimes lead to a decrease in breast volume as body fat reduces. If this is something you've experienced, this surgery can help the restoration without impacting your overall weight or shape elsewhere.

Breast Deflation

Breast deflation, or loss of fullness, can happen after pregnancy, breastfeeding, weight loss, or natural aging, which leads to reduced shape and firmness. If you wish to restore the  volume of your breasts,  this can help bring back the fullness.

Mastectomy

If you’re a breast cancer survivor who has undergone a mastectomy, the experience may have affected the way you feel about yourself. Breast augmentation can help to restore your sense of self and confidence.

Recovery After a Breast Augmentation

Scarring

A permanent scar will remain at each incision site after surgery. Dr. Jugenburg specializes in creating minimal, discreet scars that are nearly invisible. The surgical technique selected for you will consider factors such as:

  • Your natural breast anatomy
  • Personal aesthetic goals
  • The type of implant (saline or silicone)
  • The desired placement of the implant (above or below the chest muscle)ant (saline or silicone)
  • The implant’s profile and size

Risks of Complications

As with any surgical procedure, breast augmentation comes with potential risks. At the Toronto Cosmetic Surgery Institute, we take every precaution during surgery to reduce these risks and ensure the best possible recovery. Complications are rare, they can occur weeks, months, or even years after the surgery.

Current Safety of Breast Augmentation:

Silicone levels are over a million times lower than the safety limits set by FDA-reviewed studies. The amount of silicone absorbed is more than a hundred times less than the daily exposure from common consumer products that contain silicone, like antiperspirants, skin lotions, and hair care items.

Surgical Risks

As with any surgery, there are risks related to anesthesia, bleeding, infection, and blood clots. These risks are generally low, but they can occur.

Leakage

Leaks and fistulas are very rare, however can be serious if they occur. One of the most significant risks after gastric bypass is a leakage or fistula, which is the development of a small opening or connection between the newly created stomach pouch and the intestines. This can cause infection and require additional surgical intervention.

Stricture

Some patients may develop a narrowing of the connection between the stomach pouch and the intestines, leading to a condition called stricture. This can cause difficulty swallowing or eating and may require further treatment.

Dumping Syndrome

After gastric bypass, some patients may experience dumping syndrome, a condition characterized by rapid gastric emptying. This can result in symptoms like nausea, vomiting, abdominal cramps, diarrhea, and weakness after consuming certain foods, particularly those high in sugar or fat.

Nutritional Deficiencies

Gastric bypass can impact nutrient absorption, particularly for vitamins and minerals. It's important to follow a lifelong regimen of vitamin and mineral supplementation and adhere to dietary guidelines to prevent deficiencies.

Gallstones

Rapid weight loss after gastric bypass surgery may increase the risk of developing gallstones. Medications or further surgical intervention may be required to address this issue.

Long-Term Lifestyle Changes

Successful outcomes after gastric bypass surgery require significant lifestyle changes, including dietary modifications and regular physical activity. Adherence to these changes is crucial for long-term weight loss and overall health.

FAQ
What is the annual number of procedures performed by Dr. Jugenburg?
Each year, we perform approximately 500 breast augmentation surgeries and around 200 combined breast augmentations with lifts, which places Dr. Jugenburg among the top five breast augmentation specialists in the country. Over his surgical career, he has completed thousands of procedures.
Is silicone or saline implants prefered?
It's split equally between both.

We like saline if there is a rupture it will be immediately noticed. Although both saline and silicone implants are safe, effective, and have a similar appearance. The primary difference lies in feel, not look—contrary to some internet claims. In most patients with sufficient natural breast tissue, it’s impossible to distinguish between silicone and saline. We’re particularly impressed with the new Ideal Implant, a saline-based option that incorporates features of silicone, offering the best of both worlds. We’re big fans of the new Ideal Implant—it’s saline-based but has the feel of silicone, giving you the best of both worlds.
Which implants are most used?
We primarily use Allergan Inspira, which is the latest and best silicone implants available. We also offer Mentor implants for those who prefer them. Recently, we added the Ideal Implant to our lineup—a new, saline-based option that looks and feels like silicone, giving you the benefits of both types.
What if a medical emergency occurs?
As a fully certified facility, we're equipped to handle any emergency, just like a hospital. If necessary, we can transfer patients to the nearest hospital, St. Michael’s, which is only five minutes away, or to Humber Hospital, where Dr. Jugenburg has privileges and can provide continued care.
Are re-operation fees included in the surgical fee?
We stand behind our work. If you encounter any issues after your breast augmentation, we’ll take care of it at no extra charge. Re-operations needed to address deviations from the desired result, capsular contracture, or implant malposition immediately post-surgery are covered.

If an implant leaks or ruptures within the first year, it’s fully covered. After one year, the implant manufacturer’s lifetime warranty covers leaks or ruptures, with only anesthesia costs being your responsibility.

Although, the following re-operations are not included in the initial fee:
- Changing implant size (larger or smaller)
- Switching implant type
- Adjusting for breast changes due to pregnancy, weight fluctuations, or natural aging.

Please remember that your body and breasts will continue to change over time, including natural shifts in shape, volume, and position. Touch-ups for these normal changes are not covered. Over time, gravity will also cause implants to shift—downward when standing and to the side when lying down. This is normal.
What re-operation does Dr. Jugenburg perform most frequently?
When implants are positioned under the muscle, the natural asymmetry of the muscle anatomy (which is completely normal) can sometimes cause one implant to sit higher than the other. If post-surgical massage doesn’t help balance the implants, additional assistance may be needed to bring them into alignment.

Keep in mind that some asymmetry is normal, and it's unlikely that any two breasts will ever be perfectly symmetrical.
How much cosmetic work does Dr. Jugenburg do?
Dr. Jugenburg’s practice focuses almost entirely on cosmetic procedures. He constantly performs breast reconstruction for cancer patients, which is one of his true passions.
How long does it take for the implants to settle, and for me to see the final results?
99% of implants settle within three months, although in cases it can take up to a year
